  • List of Shareholders and Director of the company

  • Power of Attorney in the name of Authorised Representative (If any)
  • Details of all the employees (if any)

Premises proof of Establishment

  • Lease Deed/Rent Agreement,
  • or Any other proof of possession of Shop and Establishment like electricity bill/ property tax paid receipt/ Khata Extract certificate.
  • Front photo of the shop and Establishment covering the surroundings along with the board of the said Establishment
  • Interior photo of the premise.
